In
the tip augmentation, random spherical volume expansion could interrupt intended
direction of skin extension which should be used for lengthening and
heightening the tip of nose. So it is very important to spare the capacity of
skin expansion not to be spent for spherical volume expansion only. We
have been tried to separate this volume expansion into detail functional
components, first, random spherical expansion and second, intended linear
expansion. The spherical expansion would be useful for dorsal augmentation, and
the linear expansion would be useful for tip augmentation. We
prefer autologus fat graft as filler for healing capacity and lower
complication rate. Barbed threads are essential to support linear expansion because
it has strong immediate tissue holding power. These materials do not need
incision. When used multiple overlapped pattern, its structures act not only as
internal splints for fat graft during graft take but also as scaffolds for
consolidation of tissue remodeling. We followed 400 cases for 7 years, immediate results were more
successful than any other materials especially in the shaping of tip. However,
when we used absorbable thread material like polydioxanon, there could be a
chance of partial regression. So we recently have used combined polyprophylene
threads. As
complications, thread extrusion was most frequent in earlier period of our
trials. We experienced one case of infection being resulted from wrong
treatment of thread extrusion. Absorption of fat could be classified as
complication when preoperative counselling is inappropriate. |
