대한미용성형학회 - Oil cysts after breast fat graft Oil cysts after breast fat graft Yang, Hyun-Jin M.D., Lee, Hee-Young M.D. Baroyl & Kangnam Plastic Surgery, Seoul, Korea Oil cysts are the most frequent complication of almost fat graft surgeries. Usually small cysts are asymptomatic, big cysts over 2cm with hard capsule can be a cause of irritation, stony senses, unnatural appearance. We have been experienced smaller, lighter cysts comparing with previous articles. Fat tissues are mainly composed by triglyceride(free oil), so harvesting injury make release large amount of free oil. Free oil among living tissues slowly move and gather each other like other liquid foreign bodies. More over this, massive garft usualy would be followed by numerous fat necrotic foci for lower survival rates. We have been our best to remove these sources of complication with using Lipokit squeezing and condensation system. We not only can remove free oil 20% or more comparing with simple centrifugation, but also older, bigger fragile adipocytes well. Injected volume would decreased to 60% after condensation with our system, how ever, we can sure it would be much better if we use condensation system because of a strong selectivity. Whole complication number was smaller than silicon implanting surgeries. The Cysts are easily removed with blunt needle aspiration with just palpation without any incision. Smaller cysts could be remained with intra capsular hematoma or little remnant of oil. It also would be removed with another needle aspiration. |
